Is PMP worth listing if it has lapsed?

List it with the dates it was held. A lapsed certification is a fact; hiding it and being asked about it is a problem.

How do I show project management on a résumé if it was not my title?

Lead the bullet with the scope you actually held — budget, people, duration. Titles vary by company; scope does not.

Should I list every project?

No. Three to five that show range in size and type. A list of twenty reads as a timesheet.
